Biology / Biological Sciences
Most universities accept either Physics or Mathematics as the 4th subject. Check your specific school.
Biochemistry
Chemistry is the most important subject here — it carries the most weight at O'Level too.
Microbiology
Same combination as Biochemistry — strong Chemistry and Biology O'Level credits required.
Botany / Plant Biology
Zoology / Animal Biology
Anatomy
Physics is required (not optional) for Anatomy in most universities.

In most universities, no. Physics and Mathematics are both compulsory for Computer Science. Biology is sometimes accepted as the optional 4th subject but cannot replace Physics or Maths. Always verify with your specific school's JAMB brochure entry.
JAMB does not offer Further Mathematics as a separate subject. Mathematics in JAMB covers both regular and further maths topics. So you only register "Mathematics" — there is no Further Mathematics option in UTME.
Use the JAMB brochure on jamb.gov.ng which lists every course and its required combination. As a general rule for Science faculty: English + Mathematics + two of (Biology, Chemistry, Physics) covers most courses. When in doubt, Biology, Chemistry, Physics and Maths as a full set covers the broadest range.
You can still register Mathematics instead of Physics for Biochemistry in JAMB — most schools accept that. However, you must have Biology and Chemistry credits in your WAEC or NECO result. The Physics or Maths option in JAMB gives you flexibility, but your O'Level credits are what universities check most carefully during screening.
